Required information
Please have the following patient information available when referring a patient:
- Date of birth
- Social security number
- Referring physician
- Insurance carrier
- Diagnosis
- Patient phone number
- Reason for referral
Note for referring offices:
We will call the patient the day before the initial appointment for reminder.
